Keto LDL Cholesterol: What the Average Rise Hides From You

7 min read

Key takeaways

  • On average, keto LDL cholesterol rose modestly across the largest recent pooling of randomized trials, and the certainty behind that average is low.
  • The trials disagreed, and their people disagreed more, so your number can land far from the pooled figure.

How much does keto LDL cholesterol rise on average?

On average, a little. A systematic review in BMC Cardiovascular Disorders, published 30 April 2026, pooled 48 randomized trials reporting low-density lipoprotein cholesterol, and keto raised it by 8.22 mg/dL. It defined a ketogenic diet, the strictest low-carbohydrate high-fat diet, as over 45% fat and under 10% carbohydrate.

The GRADE rating called certainty for this result low. The next study could still move the average.

What do an average, a prediction interval and heterogeneity mean?

An average is the middle of the trials. A prediction interval is where the next trial will probably land. Heterogeneity is how much the trials disagree. Heterogeneity here scored an I² of 59.9%: most of the spread was real difference, not chance. The prediction interval ran from −1.9 to 18.9 mg/dL, so the next trial could raise nothing, or more than double the average.

Here is the real problem. Those figures describe groups, and none of them can reduce you to one.

Why can one person's number sit far from the pooled figure?

Because who you are changes the response. In the BMC review, the rise was larger in adults 50 or under, at a BMI of 30 or less, and in trials of 12 weeks or shorter. People with obesity moved less. Soto-Mota and colleagues found a substantial rise likely with low but not high BMI, a point for individual management of cardiovascular disease risk.

One published case report describes a normal-weight 38-year-old man whose number reached 496 mg/dL on the diet. That is the tail of the curve, not hyperbole. The subgroups tested factors such as age and BMI. Not sex. Not ethnicity.

Why is my LDL so high but I am healthy?

Only your own doctor can read your panel, but one pattern stands out: lean, metabolically healthy people are often the ones whose number climbs most when carbohydrate intake drops. Nobody yet knows the long-term impact. In the KETO trial, people with diet-induced levels of 190 mg/dL or more had no more coronary plaque than matched people averaging 149 mg/dL lower. The number was not associated with plaque in either group, although the study was small and cross-sectional, and it does not show that a high number is safe. The BMC authors still urge caution until long-term outcomes are in.

Most people start a keto diet for weight loss and meet this fight later. In my experience, the panic arrives first. As I wrote in the book, “someone’s cholesterol touches 200 and they start freaking out.”

Where does Dr. Mike's lipid panel sit inside that spread?

Exactly one dot. Dr. Mike, a fit doctor who ate 600 burgers in 30 days on camera, is a sample of one, and his month matched none of the trial diets: fast-food patties and water, no bun. The film shows his baseline draw at 00:13:34. It also shows his own label-based estimate of 160 grams of fat a day, 50 of them saturated, plus 700 milligrams of cholesterol.

The film suggests he expected trouble: at 00:14:09 he says, “I’m expecting inflammation panel to skyrocket.” What happened to Dr. Mike’s blood, and where his panel went, is what the film is for.

Is ketosis real, and how did I check it?

Yes. Ketosis is a measurable state, and a blood meter tells you whether you are in it. In my experience, the meter is honest and the labels are not.

In the film at 01:35:03, I set my bar at 0.5 millimoles per liter. At 01:34:56, I report that 12 grams of exogenous ketones raised my reading by 0.7. At 01:29:41, I admit they did no better than skipping food all day. The book adds around 36 hours of gastric distress. Science is glamorous.

Frequently asked questions

What is HDL cholesterol?

HDL is high-density lipoprotein cholesterol, and the BMC review lists low HDL among cardiovascular disease risk factors. Keto raised it by 3.6 mg/dL there, but a prediction interval of −5.8 to 13.0 mg/dL means some people see no change or a drop. The PubMed abstract also notes that HDL gains showed a negative correlation with trial length.

What flushes cholesterol out of your body?

Nothing flushes it; cholesterol is not sludge in a pipe. The KETO trial authors call the response to carbohydrate restriction multifactorial, including genetics, saturated fat and fiber intake. A review of normal-weight adults found baseline BMI tracked the rise more strongly than study-level saturated fat.

How often should cholesterol levels be rechecked after starting keto?

The BMC authors suggest repeat lipid tests every 4 to 8 weeks after starting, due to the wide prediction intervals. They call it personalized response tracking. That is their reading of the science, not a rule for you; your own medical team sets the schedule.

Does keto lower triglycerides as well?

On average, yes. The same review found the diet cut triglycerides by 22.31 mg/dL against control diets, with moderate certainty. Responses still vary from person to person. I covered why that marker matters in one of my earlier articles, keto and triglycerides.

What is in the book 600 Burgers in 30 Days?

600 Burgers in 30 Days is the official book of The Keto Project movie, by me and Dr. Mike. It is the resource behind the film: his journal day by day, the testing chapter and the expert interviews. It holds no lab numbers for him; those stay in the film.

The Bottom Line

Here is the test. The average describes a crowd; your panel describes you, and individual results vary. A rise is a reason to talk with your own doctor, not a verdict. Personalized medicine is boring. Boring is good.
